Home health care, general programs
Skilled Nursing
Skilled nursing care refers to a patient’s need for care or treatment that can only be performed by licensed nurses. This type of care is usually offered in hospitals, assisted living communities, Life Plan Communities, nursing homes, and other certified locations.
Physical, Occupational, and Speech Therapy
This program includes evaluations, interventions, and treatments that help patients regain function. We provide care to patients whose lives have been affected by the disease, injury, or disability. Therapists address impairments in patients’ ability to move, speak, eat, walk, dress, and otherwise participate in their normal daily lives.
Medication Administration, Management and Education
Proper drug administration can determine if patients will gain any clinical benefit and if they will suffer any adverse effects from their drugs. Medication management helps patients follow their treatment plan and avoid the adverse effects associated with incorrect medication administration.
Home Health Aide
Our trained and certified healthcare worker will assist the patient in the home with personal care (as hygiene and exercise) and light household duties (as meal preparation) and will monitor the patient’s condition.
Postoperative Care
Postoperative care is administered after a surgical procedure. The type of postoperative care depends on the type of performed surgery, as well as the patient’s health history. It often includes pain management and wound care.
Patient and Caregiver Education
Patient education can help providers inform and remind patients of the proper ways to self-manage care, understand the care setting most appropriate for their condition and avoid nonessential readmissions.
Chronic Disease and Pain Management
This program will generally encompass a wide variety of methods to manage discomfort, improve function, and boost overall health and wellbeing. Treatment plans will vary from person to person and may include medications, medical or surgical interventions if needed.
Medical Social Work
We provide the information and resources to cope with the emotional, financial, and social needs that arise with a diagnosis. As an advocate for the patient, a social worker assures that patients’ emotional and social needs are addressed through their medical treatment.
Wound Care
We cover a wide range of therapies typically needed by the wound care patient and manage the most difficult wounds. In conjunction with traditional techniques, we provide nutrition consultations and anti-infective therapy pr to further improve the healing process.
Diabetes Management and Education
This program teaches patients to manage their diabetes and related conditions. The patient learns how to eat healthily, be physically active, monitor blood sugar levels, take medication, reduce the risk for other health conditions, cope with emotional outbreaks.
Cardiac and Pulmonary Rehabilitation, CHS / COPD EHF
Heart rehabilitation and weight management
This is a supervised exercise program that involves education, nutritional counseling, stress management, smoking cessation, and other lifestyle changes designed to prevent or reverse the progression of cardiac and pulmonary diseases.

Fall Injury Prevention
Fall prevention is a variety of actions to help reduce the number of accidental falls suffered by older people. Falls and fall-related injuries are among the most serious and common medical problems experienced by older adults.
Infusion and IV Therapy
In conjunction with Coram Specialty Infusion Team, or your preferred infusion provider, we coordinate pharmaceutical needs with ordered medical care. This includes home infusion services, IV antibiotics, PICC line, pain management medication, and more
Orthopedic Rehabilitation
Our therapists design innovative protocols in physical rehabilitation to help our patients achieve optimal results. A unique pain-inhibiting home rehabilitation program is based on Proprioceptive Neuromuscular Facilitation (PNF) in conjunction with manual, physical, and occupational therapy.
Palliative Care Program
We provide relief of the pain and other distressing symptoms associated with serious life-altering illnesses to ensure the best possible quality of life for patients and their families. It is appropriate at any stage of an illness for patients of any age and can be provided at the same time as curative treatment.
Complex Wound Care (grade 3)
Our Wound Care Program covers a wide range of therapies typically needed by the wound care patient in order to manage the most difficult wounds. In conjunction with traditional techniques, we provide nutrition consultations and anti-infective therapy prescribed by the physician to further improve the healing process.
Stroke Rehabilitation
We offer a physical therapy program that includes exercises to strengthen muscles, improve coordination, and regain range of motion, and constraint-induced therapy encouraging the patient to use the affected limb to regain movement and function.
Tube Feeding Support Program
TrueMed team provides nutrition services, consultations, and personal feeding plans to home tube feeding patients. Registered Dietitians on staff provide ongoing patient education, phone nutrition consultations, and in-home nutrition visits to supervise nutrition plans for each patient based on their diagnosis, schedule, and needs.
